Membership Rules
- Treatment needs to be used before the last day of the month.
- The membership contract must be a minimum of 12 months.
- 24 hr cancellation notice is required, or service will be forfeited for the month.
- A monthly membership fee may be applied to a more expensive service.
- The credit card will be charged on the day you sign up for the membership. Subsequent charges will be charged on the first of each month.
- Member must notify us of any changes in address, contact Information and/or credit card information.
- How do I cancel my Membership?
- If you would like to cancel your membership after the initial 12-month commitment, please notify us via email {intentionalaesthetics4U@gmail.com}.
- Written cancellation of membership must be received 30 days prior to the next billing cycle.
